This privacy notice explains why we collect information about you, how that information may be used, how we keep it safe and confidential, and what your rights are in relation to this.

This privacy notice applies to personal information processed by or on behalf of Bognor Community Partnership PCN Primary Care Network (PCN).

Bognor Community Partnership PCN PCN are two practices who work together to provide integrated services to their local population.

We work in collaboration with the following organisations:

  • NHS Trusts
  • GPs
  • Voluntary Sector Providers
  • Clinical Commissioning Groups
  • NHS England (NHSE)
  • NHS Digital (NHSD)
  • Local Authorities
  • Private Sector Providers

By operating as a network, the PCN are responsible for delivering the following services:

  • Social Prescribing
  • COVID Vaccination Programme
  • Physiotherapy
  • Mental Health Counselling
  • Medicines Management

Bognor Community Partnership PCN will only collect / use the data that we require in order to provide you with the service that you need. Data is used by staff to provide direct health services to you as a patient. These direct health services can also be provided by sub-contracted health providers. We will only use or pass on information about you if others involved in your care have a genuine need for it.

Everyone working for our PCN has a legal duty to keep information about you confidential, and all receive appropriate training to ensure they are aware of their personal responsibilities and contractual obligations to uphold confidentiality.

Under the General Data Protection regulation we will be lawfully using your information in accordance with:

  • Article 6, e) processing is necessary for the performance of a task carried out in the public interest or in the exercise of official authority vested in the controller;”
  • Article 9, (h) processing is necessary for the purposes of preventive or occupational medicine, for the assessment of the working capacity of the employee, medical diagnosis, the provision of health or social care or treatment or the management of health or social care systems

We are committed to protecting your privacy and will only use information collected lawfully in accordance with:

  • Data Protection Act 2018
  • The General Data Protection Regulations 2016
  • Human Rights Act 1998
  • Common Law Duty of Confidentiality
  • Health and Social Care Act 2012
  • NHS Codes of Confidentiality, Information Security and Records Management

We will use data which you cannot be identified from when we are undertaking the planning and commissioning of local health and care services. This data is anonymised in accordance with the Information Commissioner’s Office Code of Practice.

If you are not happy for your health data to be shared with the organisations listed above, you should contact your registered practice so they can discuss the potential impact this could have on your care and treatment. If you do not wish for your anonymised data to be used for the planning of PCN services, you are able to opt-out of this via the National Opt-Out form online.
